Apply for Healthy Montana Kids (HMK) Plan
Please note:
This application is for Montana residents only.
If more than six people live in your home, you must apply for the Healthy Montana Kids Plan by mail or by visiting your county Office of Public Assistance.
You do not need to register to apply for HMK on-line. However, if you do register, you will be able to save your application and go back to it at a later time to complete it or to make changes to your information.
The application will “time out” after 30 minutes of inactivity and all entered information will be lost. If you are a registered user, you can save the information entered if you have to step away from your computer.
Occasionally, the screen will refresh while you are filling out the application. This is normal.
If you get a security warning message, click OK. The HMK on-line application is on a secure server.
The application is divided into steps to make it easier for you to complete. Each page has controls to go to the next question, back to the previous question, or to close the form. You can quit at any time by closing the form and the system will prompt you to cancel, save, or submit your application.
In order to save your information, you must Register.

Before you begin:
Be sure you have the social security numbers for the children you are applying for.
Be prepared to enter all household income from employment and any other sources.
